FORKLIFT FAILURES: ARROW FINED $140k

Arrow Worldwide, a company providing warehousing, storage, and transport services, faced legal charges after an incident where a delivery truck driver suffered severe injuries. The driver was struck by steel beams that fell from a forklift during loading.

The forklift driver was adjusting the steel beams on the truck while the injured worker was nearby. After adjusting the steel beams on the truck’s flatbed, the forklift moved away, causing the beams to fall hitting the driver's head and crushing both his legs.

The driver sustained life-altering injuries, including the loss of both legs and a traumatic brain injury. He underwent multiple surgeries and was placed in an induced coma for a week.

Investigations revealed the company had failed to demarcate an exclusion zone to ensure drivers were segregated from forklifts during vehicle loading.

Despite the company's prior good record and charitable contributions, the court imposed a fine of $140,000, emphasizing the seriousness of the offense, along with almost $7,000 in costs.
