WORKSAFE VICTORIA ISSUES ALERT ABOUT HIGH-SPEED BEARING FAILURE

A haul truck operator at an underground mine was hauling materials when flames began in the truck’s engine bay area. The installed Aqueous Film Forming Foam (AFFF) fire suppression system failed to extinguish the fire. A portable 9kg fire extinguisher was then successfully used to end the crisis.

The cause of the fire was an overheating high-speed bearing next to the articulation joint. An investigation found that the AFFF did not cover the bearing, and the general-purpose grease recommended by the supplier was not suitable for high-speed bearings. 

Worksafe Victoria has issued an alert reminding mine operators about the risks of high-speed bearing failure on mobile equipment.  

The alert includes details of the safety issues, recommended ways to control the risks, the employer’s legal duties and a reminder of the employer’s duty to consult with employees and independent contractors.
