COMPANY CHARGED AFTER 2024 BULK BAG CRUSH FATALITY

Following the death of a worker at a Tottenham warehouse in May 2024 WorkSafe has now charged a freight transport company.

Cargo Freight Services Melbourne Pty Ltd faces two charges under section 21(1) of the Occupational Health and Safety Act for failing, so far as reasonably practicable, to provide and maintain a safe working environment.

It is alleged that the company failed to maintain a safe system of work that reduced the risk of stacked flexible intermediate bulk containers becoming unstable and collapsing onto workers, and that the company also failed to provide the necessary information and instruction to employees on the risks associated with stacking flexible bulk containers and how to do so safely.

The 63-year-old worker was attempting to repair a ripped bag of agricultural feed when three one-tonne bags stacked on top of it fell and crushed him. The man was pronounced dead at the scene.
